Topics: An introduction to co-morbidity. Rehabilitating complex patients with respiratory problems, diabetes and heart failure. Managing complex patients in the community. Coping strategies for patients. A conversation with an 'expert patient'. Prescribing issues and managing polypharmacy. How GP Practices can implement the 3D method of managing multi-morbidity. (This course follows on from the morning held on 20th November 2018. It is course in its own right, and does not require you to have attended on the 20th).
